Permanent Full-Time Position

The Flexihealth Home Care Packages Program at Bass Coast Health is expanding and requires an experienced Case Manager to join our team. The role of the Case Manager is to provide overall care coordination of packages/services in collaboration with clients to help them remain living independently in their own homes with the appropriate supports.

To be considered for this role, you will have:
• Diploma of Case Management and/or Community Services / or related field, or Tertiary Qualification in a health related field
• Experience in managing a caseload of clients using consumer directed care principles, a brokerage model of service delivery and evidenced based best practice
• Demonstrated understanding of case management, comprehensive client assessments and care planning, review and discharge/transition
• Demonstrated understanding of care coordination, documentation, and consumer budgets
• Experience in working in community aged care services and knowledge of the range of issues experienced by people living at home
• Current Victorian Drivers Licence
• A valid Police Check
