District Nursing and Palliative Care

The Bass Coast Health District Nursing & Palliative Care Service is available to community members living within the Bass Coast Shire.

The District and Palliative Care Nursing Service is an accredited service. Referrals are accepted from self, General Practitioners, Hospitals and other service providers with client consent to access both the District Nursing and the Palliative Care Service.

District Nursing Service

Bass Coast Health District Nursing Service provides a range of nursing specific services for people in their own home to optimise independence, health and wellbeing outcomes. The District Nursing Service recognises that every individual has different needs inclusive of cultural diversity.

The District Nursing Service provides care that is tailored to client needs and is delivered by a team of trained, competent and experienced Registered and enrolled Nurses. The District Nursing team is the same team that provides the Bass Coast Health Palliative Care Service (see below).

The District Nursing Service can provide

  • Wound care management, continence care, support and monitoring for client medication management, client support in stoma care, managing diabetes, and comprehensive assessment for client needs and referrals in close consultation with the client. These types of care are often provided for clients eligible for the Victorian ‘Home and Community Care’ (HACC) Program and Veteran Affairs clients.
  • Nursing services for clients receiving cancer care for symptom management, dressings for treatment devices such as Peripheral Inserted Central Catheters and disconnection of chemotherapy can be provided in close liaison with the treating chemotherapy unit and doctor.
  • In-home nursing support for clients discharged from hospital who may need short- term follow up for nursing care, referred to as ‘Post-Acute Care.’
  • Home nursing visits for Bass Coast Health’s ‘Hospital in the Home’ service and for metropolitan and regional ‘Hospital in the Home’ services for acute in-home nursing care whilst they remain an inpatient of the referring hospital.

A Doctor’s or health professional’s referral is preferred. Completion of form is required.

Palliative Care Service

Bass Coast Health Palliative Care Service is about living well, with the focus on optimising client comfort and independence. The Palliative Care Service provides holistic care that is tailored to individual needs inclusive of cultural diversity. The Palliative Care team, support the client, carer and family through their journey.

The Palliative Care service:

  • Will aim to enhance quality of life, and may also positively influence the course of illness.
  • Is applicable early in the course of illness, in conjunction with other therapies that are intended to prolong life, such as chemotherapy or radiation therapy, and includes those investigations needed to better understand and manage distressing clinical complications.
  • Integrates the psychological and spiritual aspects of patient care.
  • Affirms life and regards dying as a normal process.
  • Offers a support system to help patients live as actively as possible until death.
  • Provides relief from pain and other distressing symptoms.
  • Intends neither to hasten nor postpone death.

The Palliative Care Nurses can:

  • Provide access to limited out of hours nursing service 7 days a week
  • Support and provide education to clients and carers.
  • Liaise closely with family and carers.
  • Liaise closely with other healthcare providers such as doctors, occupational therapists, social workers, and dietitians.
  • Provide access to consult with a specialist palliative care physician if needed.
  • Arrange equipment to provide safe and more effective care of the client in their own home. Costs will be discussed as applicable
  • Organise hospital /hospice respite if needed.
  • Arrange access to trained volunteers who can provide support and friendship to client and carers.
  • Follow up bereavement visits and referral for grief counselling as required.
